Job Summary:

Under the general supervision of the COO, the primary responsibility of the Population Health Manager serves as the designated clinical leader for Camino’s Population Health Program. A key role of this position is to build strong and collaborative relationships with physicians, advanced care practitioners, care team members, and payers. The Population Health Manager is responsible for overseeing clinical quality performance and population health outcomes. The Population Health Manager will champion population health strategies to support Camino’s annual goals and objectives in the value-based healthcare environment.

Minimum Position Qualifications:

  • Education: Bachelor’s Degree in Nursing or Completed an accredited LVN program

    • Experience working with Electronic Health Records

Job Summary:

The Registered Dental Assistant’s (RDA) primary job function is to work as part of a team to provide high quality, efficient and service oriented patient care while demonstrating the health center’s core values.  The RDA is responsible for performing basic supportive chair-side procedures under the direction of the Staff Dentists, maintaining the operatories in a safe and workable condition, properly cleaning all dental instruments, and assistance with providing oral health education to patients and caregivers.  The registered dental assistant is charged with providing positive, supportive and caring behavior in all communications to patients and families.  The RDA is responsible for mentoring and supporting Dental Assistants in the essential functions associated with Direct Patient Care and Back Office Duties.

Job Summary:

Under the general supervision of the Quality Improvement Director and in collaboration with the Chief Medical Officer, the primary responsibility of the Quality Improvement Manager is to assist in the overall organization, implementation, and administration of Camino Health Center’s Quality Improvement Program and associated initiatives.

Quality Improvement responsibilities include researching best practices, developing new or enhancing existing quality improvement activities, implementing new workflow processes, facilitating trainings to health center personnel, monitoring progress towards achieving desired quality goals. Ensuring data integrity and accurate analysis of internal and external data reporting as well as reporting outcomes to applicable stakeholders as requested. The Quality Improvement Manager will assist in the implementation and any ongoing compliance requirements for the Patient Centered Medical Home (PCMH) designation. This includes collaboratively working with physicians, registered nurses, and medical assistants.

Job Summary:

The Associate Behavioral Health Specialist will work under the direct supervision and training of the Behavioral Health Director. The Associate Behavioral Health Specialist provides behavioral health services, including conducting complete brief and focused assessments utilizing the biopsychosocial approach, using evidence-based brief therapy, being flexible, and practicing in a fast-paced clinic environment. The Associate Behavioral Health Specialist work alongside the health center’s medical teams to provide comprehensive whole-person care. They will offer same-day mental health consults, psychosocial assessments, crisis intervention, psychotherapy, alcohol, and drug screening using the SBIRT (Screening, Brief Intervention, and Referral to Treatment), education/support groups, and resource information. The Associate Behavioral Health Specialist is the bridge between Camino Health Center, the community, and other local agencies that provide this population’s services.

Minimum Position Qualifications:

  • Education: Master’s degree in Social Work from a program accredited by the Commission on Accreditation of the Cultural Humility UDS Chronic Management Council on Social Work Education (CSWE).
  • License / Certification: Valid Associate Clinical Social Worker (ACSW) license issued by the California Board of Behavioral Health Sciences is required. It must be maintained to continue recording experience hours until licensed as an LCSW—current BLS certification.
  • Experience / Training: 1-2 years of experience providing direct behavioral health services to patient of all ages; ability to work through brief patient contacts as well as to make quick and accurate clinical assessments of mental and behavioral conditions; should be comfortable with the pace of primary care, working with an interdisciplinary team, and have strong communication skills

  • Experience / Training: Bilingual English/Spanish; experience in and out-patient or community mental health setting; previous experience with motivational interviewing, crisis intervention, problem-solving therapy, cognitive behavioral therapy and solution-based counseling

Job Summary:

The Dental Assistant’s primary job function is to work as part of a team to provide high quality, efficient and service oriented patient care while demonstrating the health center’s core values.  The Dental Assistant is responsible for performing basic supportive chair-side procedures under the direction of the Staff Dentists, maintaining the operatories in a safe and workable condition, properly cleaning all dental instruments, and assistance with providing oral health education to patients and caregivers. Job Summary:

Direct Patient Care: Provide high quality, patient-centered and values-based medical care, referral of patients for specialty care as needed, collaboration with health care team members as appropriate for consultation, referrals and follow-up care (80%).

Supervision of Clinical Staff: Assist mid-level health care providers in assessment and treatment of patients as necessary and direct supervision of nursing staff (10%).

Patient Education: Direct patient education regarding medical conditions and treatment, appropriate verbal and written patient referrals for further health education, assist CEO and CMO in program development and implementation as necessary (5%).

Quality Management and Utilization Review: Practice in an efficient, cost effective manner, participation in department and clinic-wide meetings, attend relevant Continuing Medical Education courses (5%).

Job Summary:

Primary job function is to work as part of a team to provide high quality, efficient service oriented patient care while demonstrating the health center’s core values. Under the direct supervision of the Clinic Manager, the Patient Services Representative (PSR) is responsible for providing a high level of customer service to Camino’s patients, community partners, and other health center employees. The PSR is responsible for effectively communicating with patients for the purpose of obtaining and relaying accurate information needed for processing their medical visit. The PSR is charged with answering phone calls from patients and the community and appropriately processing each request. The PSR is responsible for processing medical records and other patient related documents. The PSR uses the health center’s electronic health record (EHR) and electronic workflow processes to conduct their work.

Job Summary:

The medical assistant’s primary job function is to work as part of the primary care team to provide high quality, efficient and service oriented patient care while demonstrating the health center’s core values. The medical assistant works alongside physicians, mid-level providers, behavioral health professionals, registered nurses and other support staff support to delivery direct patient care services. The medical assistant is responsible for executing standing orders as well as anticipating the needs of the care team in order to help facilitate the delivery of quality clinical care and a positive patient experience. The medical assistant is responsible for actively participating in the health center’s quality and continuous improvement activities.

Job Summary:

The Behavioral Health Specialist provides behavioral health services to patients of all ages at Camino Health Center. The Behavioral Health Specialist work alongside the health center’s medical teams to provide comprehensive whole person care.
